Department spokesperson, Kwanele Ncalane, says they were informed by people at the scene that the taxi failed to come to a stop.
"We are told by those who were at the scene from the Santaco leadership, that they usually conduct an operation where they voluntarily stop vehicles. And they say this taxi they tried to stop it, but the river did not stop because he knew he was overloaded," he said.
More than a dozen others - mainly school children - were also injured.
