SA Navy to establish inquiry into submarine deaths

The South African Navy insists all safety measures were in place before this week’s deadly incident involving the submarine SAS Manthatisi.

The submarine was off Kommetjie in Cape Town on Wednesday when high waves swept seven crew members out to sea.
Three of the crew died, including Lieutenant Commander Gillian Hector.
Hector was the first woman in Africa to navigate a submarine.

"Fortunately, others were not recovered back to the submarine but were recovered by boat from NSRI [National Sea Rescue Institute]," says the Navy's Musawenkosi Nkomonde.
"All safety precautions were in place, which now makes this accident a mystery for us. Hence, we are going to conduct a board of inquiry to establish what really happened."
