Authorities in Mpumalanga are investigating the exact cause of the accident, but dangerous overtaking is suspected.
Later in the morning, a tow truck operator died in a collision in Verulam, north of Durban.
First responders say the incident occurred on Old Main Road in Ottawa earlier.
"We were informed of a tow truck that had collided with a barrier,” said Reaction Unit South Africa's Prem Balram.
“Paramedics and Reaction Unit got to the scene, and they found that the tow truck driver had demised on the scene.
“He was declared deceased by paramedics a short while later. No other vehicle was involved in the collision, and the cause of the accident was not immediately established."
