Emergency Services say a taxi plunged off a bridge and crashed onto a railway line below.
The minibus was then hit by a train.
Netcare 911's Chris Botha says the accident happened near the N2 highway.
"The vehicle actually broke up and we found accident debris and people spread over quite a large area. One person was found in a critical condition and 6 others sustained very serious injuries.
"SAPS is on the scene as well as the fire department and the various medical emergency services. This will remain a problem for quite some time until investigations are concluded," he said.
UPDATE: The death toll has risen to 15.
