A family of five, including a mum, dad and their children, were killed when a bakkie and a car collided in Gingindlovu.
Seven others, including four children, suffered serious injuries.
The department's spokesperson, Ndabezinhle Sibiya, says they received the first report into the R66 crash on Sunday night.
"According to RTI, the Hyundai Grand i10 was transporting eight passengers. They were travelling from Nongoma to Durban. Three other passengers from the same vehicle survived.
"In a second vehicle, a white Ford Ranger LDV, there were four passengers who survived. They were travelling towards Gingindlovu.
"Preliminary reports indicate that the Ford Ranger LDV lost control and collided head-on with the Hyundai Grand i10."
The victims were travelling in the car.
